Providing care to individuals with serious mental illness can be challenging, requiring ongoing monitoring, treatment adjustments, and coordination of various medical and social services.. This study aims to assist with coordination of passive mobile monitoring of Veterans' mental health status, technology and behavioral supports from peer coaches, and care coordination within a Behavioral Health Interdisciplinary Program (BHIP).
Mental Health Care shouldn't feel like you're climbing Mount Everest. The aim of this study is to provide Care Coordination and Mobile Data Monitoring to Engage Veterans and Improve Mental Health Care in the long term for patients and their families.
